Service Description

At Rising Together Healthcare, we focus on providing specialized care for each patient. During your new patient appointment, we will take the time to discuss your medical history, current health goals, and any concerns you may have. From there, we'll work together to create a personalized treatment plan that addresses your unique needs. Please note that Rising Together Healthcare is not a primary care service provider. We do not offer primary care services, but we will coordinate with your primary care provider as needed to ensure you receive the best possible care. We will also coordinate with other members of your care team as requested. Patient Care Pricing: New Patient Initial Visit - 60 minutes (50 minutes visit, 10 minutes care coordination): $600 Additional time (if available) billed in: 15-minute increments ($125/15 min) Form Completion: $25/form; includes prior authorizations

Cancellation Policy

Once your appointment has been confirmed, you should expect 2 emails; an email from the electronic health record with intake forms (if applicable) and a calendar invitation with the physical address location (if visit is in person on Thursday morning) or virtual link (if visit is at any other time). At Rising Together Healthcare, PLLC, we require at least 24 hours notice if you need to cancel or reschedule your appointment. If we do not receive sufficient notice, a missed appointment charge of $100 will be applied. However, we understand that unforeseen circumstances arise and we offer a one-time waiver of this charge per year. Our cancellation policy is in place to ensure that all of our clients receive the care and attention they need, while allowing us to effectively manage our schedule.
