Former Patient Initial Visit

Service Description

If you are a former patient looking to re-establish care with Dr. Monge at Rising Together Healthcare, please book an initial visit here. During this appointment, we will discuss your current health goals and any changes to your medical history since you were last seen by Dr. Monge. Please note that Rising Together Healthcare is not a primary care service provider. Dr. Monge no longer provides primary care services, but she will coordinate with your primary care physician as needed to ensure you receive the best possible care. Patient Care Pricing: Former Patient Initial Visit - 45 minutes (40 minutes visit, 5 minutes care coordination): $375 Additional time (if available) is billed in 15-minute increments ($125/15 min) Form Completion: $25/form; includes prior authorizations

Cancellation Policy

Once your appointment has been confirmed, you should expect 2 emails; an email from the electronic health record with intake forms (if applicable) and a calendar invitation with the physical address location (if visit is in person on Thursday morning) or virtual link (if visit is at any other time). At Rising Together Healthcare, PLLC, we require at least 24 hours notice if you need to cancel or reschedule your appointment. If we do not receive sufficient notice, a missed appointment charge of $100 will be applied. However, we understand that unforeseen circumstances arise and we offer a one-time waiver of this charge per year. Our cancellation policy is in place to ensure that all of our clients receive the care and attention they need, while allowing us to effectively manage our schedule.
